Everyone participating in club activities off campus must first sign an indemnity form (see below for the form in PDF and also the original MS Word document). Currently this must be done for every trip you participate in. Next year we hope to be able to have people sign it once.

Trip leaders, could you please print out enough copies of this form for everyone on your trip and get people to sign it before the trip. The completed forms should then be taken to the gear store where they will be archived. There is a lever arch file on or under the desk for this purpose.
